What is a death certificate?

A death certificate is an official document that confirms a person’s death. It typically contains details such as the deceased’s name, age, date of death, place of death, and the medically certified cause of death where applicable.

In Nigeria, death registration falls under the responsibility of the National Population Commission (NPC), which manages the country’s Civil Registration and Vital Statistics system.

According to the NPC, death registration is compulsory under Nigeria’s civil registration framework.

Step 1: Obtain medical confirmation of death

When a person dies in a Nigerian general hospital, a doctor must first confirm the death.

The hospital will usually issue a medical certificate or death notification document stating that the death occurred within the facility. This document is important because it serves as the primary evidence required for official registration.

Families should ensure that all details on the hospital-issued document are accurate before leaving the hospital.

Step 2: Visit the appropriate registration office

After obtaining the hospital’s death notification, the next step is registering the death with the National Population Commission.

The NPC operates registration centers across Nigeria’s local government areas for civil registration services.

The registration can be initiated through the NPC’s death registration platform or at designated registration centers.

Step 3: Provide required information

The person reporting the death, often a family member or next of , may be asked to provide:

  • Full name of the deceased
  • Date of birth, if available
  • Date of death
  • Place of death
  • Cause of death from the hospital record
  • Residential address
  • National Identification Number (NIN), where available
  • Informant’s identification details

The NPC’s online death notification system currently requires informants to provide their NIN during registration. And at the online process, there are four steps to go through. The particulars of the deceased, particulars of the informant, documents upload, and then information review.

Step 4: Complete death registration

Once the information has been verified, the registration process will be completed by the authorized registration officer.

The National Population Commission stated that death registration forms part of Nigeria’s Civil Registration and Vital Statistics system, which records important life events such as births and deaths.

Accurate registration can help government agencies maintain population records and generate public health statistics.

Step 5: Collect the death certificate

After successful registration, the death certificate can be issued through the appropriate registration authority. So, families should carefully check the certificate before collection to ensure that names, dates, and other personal details are correct.

Any error discovered after issuance may require an additional correction process.

Also, the certificate can be required to access pension benefits, settle insurance claims, close bank accounts, update official government records, handle property ownership transfers
